Our Admission Process

Move in times are weekdays from 08.30 to 14.00

Individuals joining Graceland Nursing Home from a private residence.

The following is required:

A full medical report from the individual’s doctor including the medical history of the individual and any specific care needs where applicable,
A current medication script,
Copy of the individual’s ID,
Copy of the individual’s medical aid card (if applicable),
Copy of the ID of the individual responsible for the account,
Signed contract

Individuals joining Graceland Nursing Home from a hospital, step-down or rehabilitation facility.

The following is required:

A full medical report from the individual’s doctor including a medical report from the hospital confirming the individual’s diagnosis, continued therapy requirements, latest laboratory reports (where applicable),
A current medication script,
Copy of the individual’s ID,
Copy of the individual’s medical aid card (if applicable),
Copy of the ID of the individual responsible for the account,
Signed contract

Individuals joining Graceland Nursing Home from another nursing home or retirement home/centre.

The following is required:

A full medical report from the individual’s doctor including the medical history of the individual and any specific care needs where applicable,
A progress report from the transferring facility,
A current medication script,
Copy of the individual’s ID,
Copy of the individual’s medical aid card (if applicable),
Copy of the ID of the individual responsible for the account,
Signed contract,
